Western Victoria Primary Health Network is one of 31 Primary Health Networks (PHNs) across Australia and one of six in Victoria. We are a not-for-profit organisation, membership based, company limited by guarantee, and were established on 1 July 2015. Each part of the region has established Regional Integrated Councils (with both clinical and community members) to provide the organisation with insights into key regional healthcare needs. Our Board is skills-based and made up of four elected and five appointed Directors.
Our position statement
Together with our partners and communities, Western Victoria PHN identifies priority health care needs, improves access through government funding, and co-designs localised solutions to improve health care systems across western Victoria.
What we do
Western Victoria PHN regional centres are hubs of innovation, and we work in partnership with government, community organisations, general practice, primary care partnerships, Aboriginal health services, private allied health providers including pharmacies, private hospitals, health funds, aged care, universities, research alliances, regional GP training providers, rural workforce agencies and the many public rural and regional hospitals across the region. Our regional centres share knowledge and work collectively to meet national, local and organisational indicators, as part of the PHN performance framework.
We are funded by the Federal Government to work in particular across the primary care sector and its interface with the secondary and tertiary care sectors to improve health outcomes. The key objectives set for PHNs by the Federal Government are:
To increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes
To improve coordination of care to ensure patients receive the right care, in the right place, at the right time
