    Primary Health Networks are outcomes focussed, independent, regionally based planning and commissioning organisations that play an important role in the delivery of primary health care across Australia. They perform essential functions such as connecting health services to support consumers to access the care they need as close to home as possible.

    Population Health Snapshot

    PHNs have two key objectives for the work funded by the Australian Government:

    • to improve the efficiency and effectiveness of health and medical services for patients, particularly those at risk of poor health outcomes; and
    • to improve the coordination of care to ensure patients receive the right care, in the right place, at the right time. 

    The Australian Government has identified seven priority areas to guide the work of PHNs. The priority areas are:

    • Mental Health
    • Aboriginal and Torres Strait Islander Health
    • Population Health
    • Health Workforce
    • Digital Health
    • Aged Care
    • Alcohol and Other Drugs

    As an independent organisation, Western Victoria Primary Health Network is free to focus on areas and issues beyond the seven priority areas.
