Connect Back Resources Package

It can be difficult to find the right level of support in the right place at the right time. The Connect Back Resource was developed by Wellways to support the community to better navigate help when it is most needed. This resource is a guide to navigating supports that suit an individual’s unique circumstances.

The tool developed is not intended to substitute professional support or treatment, but it can support people to keep on track and moving forward.

The Connect Back Resource was created in consultation with people with lived experience, mental health service providers and community members to determine how best to support a person along their journey...

The tool identifies different supports that are available, how to access them and use them most constructively. This resource enables people to make their own choices about self-care and supports recovery by providing guidance.

The Connect Back Resource Package can be found at community based organisations.

A printable PDF version will be available, once available you will find it in the documents section of this page.
