Advance Care Planning Education Calendar
As part of the Greater Choice for At Home Palliative Care Project, Western Victoria Primary Health Network (WVPHN) aims to build Advance Care Planning awareness and capability across our region.
WVPHN has identified existing Advance Care Planning workshops within our region whilst also establishing workshops in gap areas with assistance from expert members of our project advisory group.
We are currently in the process of planning our 2025 Advance Care Planning workshops. If you are interested in attending a future session, please contact us at palliativecare@westvicphn.com.au.
