Starting April 2024, WVPHN are embarking on an important project to enhance multidisciplinary team care in general practice for people living in the Wimmera Grampians region, which based on available data is the area with greatest need for multidisciplinary services in our region.
Here's what's in store:
WVPHN is about to consult with rural general practices and other health professionals, including allied health providers, community health workers, bush nursing workers and Aboriginal Community Controlled Organisations (ACCOs) within the Wimmera Grampians region to improve our understanding of the specific service needs, barriers, and challenges in delivering multidisciplinary team care and develop a plan to improve access to multidisciplinary services in small rural primary care setting.
Following the consultation, an online market sounding session to introduce and discuss designed service models will be facilitated to seek your feedback to inform service models that WVPHN may commission with the Wimmera Grampians region. Our goal is to increase patient access to multidisciplinary services and improve the management of chronic conditions.
